Information, not treatment

Content on this site, including our guides and blog, is general information. It is not a diagnosis, medical advice or a substitute for care from a qualified professional. Visiting this site, calling us or sending a website message does not create a provider–client relationship.

A provider–client relationship begins only after we complete our intake process and you and your provider agree to begin services. Clinical services are governed by your intake and consent documents, our Notice of Privacy Practices and the terms of our patient portal.

In an emergency

This website and our office are not crisis services. If you are in crisis, call or text 988 (Suicide & Crisis Lifeline), or call 911 in an emergency.

Do not use the website contact form or ordinary email to report a crisis or send urgent health information.

Clinical services

Assessment and counseling are provided under separate clinical consent and intake documents. Scheduling, intake forms, secure messages and billing take place in our patient portal, SimplePractice, which has its own terms and privacy policy.

The website does not provide access to your clinical record. If you need records or have a question about your care, contact our office directly.

Appointments, cancellations and fees

Requesting an appointment online or by phone does not confirm an appointment until our office confirms it. Please give us at least two business days’ notice to cancel or reschedule. Late cancellations and missed appointments may be charged a fee.

Current details about insurance, self-pay testing, payment options, other services and cancellations are on our Insurance & fees page. Call us at 817-735-4165 if anything is unclear.

Accuracy of information

We try to keep the site accurate and current, but information can change. We do not guarantee that every page is complete, current or suitable for your situation. Content may be updated, replaced or removed without notice.

Do not delay seeking professional advice because of something you read on this site.

Intellectual property

Text, images, design, logos and other content on this site belong to Fair Winds or are used with permission. You may view, print or share a link to our pages for your own personal, noncommercial use.

You may not copy, republish, sell, license or use our content for commercial purposes without written permission. Short quotations with a link back to the original page are fine.

Acceptable use

You agree not to:

Use the site for an unlawful, infringing, harassing or harmful purpose.
Upload or transmit malware, or attempt to gain unauthorized access to the site or its systems.
Impersonate another person or misrepresent your connection to us.
Interfere with the site or place an unreasonable load on it.
Use automated scraping or bots that ignore our robots.txt or disrupt service.
Collect information about other visitors.
Use the site to send health information or records through an insecure channel.

We may suspend or block access if we believe these rules have been violated.

Third-party sites and services

We link to the patient portal, payment and financing sites, Google Maps, our Google business listing and Facebook for your convenience. Those sites are not under our control and have their own terms and privacy policies. We are not responsible for their content, availability or practices.

Privacy

Our Privacy policy explains how we handle website information. Our Cookie policy explains the cookies and browser storage we use. Our Notice of Privacy Practices explains how we protect health information.

Disclaimers

The site is provided “as is” and “as available.” To the extent permitted by law, we make no warranties, express or implied, about the site, including warranties of merchantability, fitness for a particular purpose, accuracy or uninterrupted availability.

Nothing in these terms limits any rights you have under applicable law, including rights that cannot be waived, or our professional and legal obligations to you.

Limitation of liability

To the extent permitted by law, Fair Winds will not be liable for indirect, incidental, special, consequential or punitive damages, or for lost data, profits or goodwill, arising from your use of or inability to use this website.

This limitation does not apply to liability that cannot be limited by law, to professional services, or to any rights you have under our clinical agreements or the Notice of Privacy Practices.

Governing law and venue

These terms are governed by the laws of the State of Texas, without regard to conflict-of-law rules. Any dispute about this website will be brought in the state or federal courts located in Tarrant County, Texas, unless applicable law requires otherwise.

Changes to these terms

We may update these terms as the site or our services change. We will post the new effective date at the top of this page. Continued use of the site after an update means you accept the revised terms.

Severability

If any part of these terms is found unenforceable, the rest will remain in effect. The unenforceable part will be limited or removed only to the extent necessary.
