Records and reports
How to request a copy of your record or an evaluation report, and what to expect.
How to request records
Records must be requested in writing. You can submit a request through the secure patient portal or complete a form in our office.
A full chart pull can take upwards of one week, depending on the current backlog. Rush requests can be ordered for an additional fee.
Open the patient portalOpens SimplePractice in a new tab.

- What you need: the full record, a specific report, a letter or a summary
- The dates or providers involved, if you know them

- A signed release if the records go to someone else
- Whether you need a rush request

Who can request
Adults can request their own records. A parent or legal guardian can request records for a minor. For an adult who needs support, a signed authorization or legal authority may be required.
