Insurance and fees, explained plainly
Here's how insurance, self-pay testing and payment work at Fair Winds, and why a testing cost can't be set until your provider knows what's needed.
Covered by insurance
We accept most major insurance for the initial evaluation and for counseling before and after testing.
- Aetna
- Blue Cross Blue Shield
- Baylor Scott & White Health Plan
- Cigna
- Evernorth
- Magellan Health
- MHN

- UnitedHealthcare
Plans change often. Please call us to confirm your coverage before your first visit.
Testing is self-pay
Insurance companies often limit how much testing can be done and which tests can be used. We won't lower the quality of an evaluation to fit those limits.
Testing costs vary a great deal from person to person. Until your provider completes the diagnostic interview and decides which tests are needed to answer your question, there's no accurate way to price it, so we don't quote testing costs over the phone.
You can ask for an estimate at any time. We'll give you one for the diagnostic interview, then a new written Good Faith Estimate for testing once your plan is set, before testing begins.
Two testing paths, two different scales
Your provider recommends a path after the diagnostic interview. If a focused test can answer your question, you won't be asked to complete a full battery.
Full battery
For autism, ADHD, learning, language and developmental questions, or when several concerns overlap.
- A detailed history and forms from home, school and others
- 2 to 6 testing sessions of about 2 hours each
- Look-alike and co-occurring conditions ruled in or out
- A comprehensive report, usually 20 to 75 pages, and a feedback session
Focused diagnostic testing
For a narrower question, such as anxiety, depression or a personality disorder, when one test or a few can answer it.
- A diagnostic interview to define the question
- One or a few targeted measures
- Generally shorter, less intensive and less expensive
- A written summary with treatment recommendations
Ways to pay
- Cash
- All major credit cards
- CareCredit financing, so you can spread the cost of testing over time.
Financing with CareCredit
CareCredit is a health care credit card. You can apply online and see whether you prequalify before your visit.
The calculator is provided by CareCredit and loads from their website only if you choose to open it. Subject to credit approval.
Paying a bill
Use quick pay to pay a balance by card without logging in, or open the patient portal to see statements and your full account.

Other services and fees
These services are billed separately from testing and counseling. Fees vary with the time involved, so please call our office for current fees.
- Court and legal services
- Court testimony, depositions, court-ordered or legal reports, and consultations with attorneys.
- Disability paperwork
- Forms and letters for SSI, SSDI and other disability applications.
- Emotional support animal (ESA) letters
- An evaluation and, when clinically appropriate, a letter documenting the need for an emotional support animal.
- Records and reports
- Copies of records, additional report copies, rush reports, and school or agency forms.
Cancellations
Please give us at least 2 business days’ notice if you need to cancel or reschedule. For a Monday appointment, that means letting us know by the same time on the Thursday before. Late cancellations and missed appointments may be charged a fee.
Testing appointments reserve several hours of a provider's time, so advance notice lets us offer that time to another family who is waiting.
Wondering what a full evaluation might show?
Every person is different, and so is every evaluation. Tell us what you're noticing and we'll help you decide whether a closer look makes sense.
