We have recently moved to an Electronic Health Record program that will handle all documentation, forms, billing and scheduling. If you have already setup your access to the portal or if you are a new patient looking to schedule online instead of over the phone click here to access that portal.

Downloadable Forms
We understand your time is valuable and filling out forms before an appointment can be tiresome. To make sure your experience of setting up an appointment to your visit is a pleasant one we have conveniently uploaded our documents here to our site so you can download them, print them out, and fill them out in advance in your leisure. This way you do not feel rushed on the day of your appointment and you can take advantage of the time allotted for your appointment.
Your new patient appointment here at Fair Winds Assessment and Counseling Center will introduce you to your therapist and properly assess your needs. We strive to provide the best experience by providing a television playing child-approved movies, current and interesting magazines, and a cozy atmosphere that doesn't feel like a doctors office.
If there are any questions regarding your appointment or mental health insurance coverage we welcome you to contact our friendly staff. During this time since we are not currently seeing patients face-to-face please email your paperwork completed to info@fairwindsmentalhealth.com. Please do not send any additional personal information or request appointments through e-mail as e-mail cannot be guaranteed HIPAA compliant. We ask that you send the forms as PDFs. If you do not have access to a scanner to scan the documents there are many apps available on each respective app store that can scan and email documents. Check our our blog post to help.
